This page is for customers who have already ordered a HughesNet system from an EZTV Sales Agent or Mobile Music. During the Hughesnet Installation process, the customer is required to agree to the HughesNet ™ Subscriber Agreement before the installation can be completed. EZTV requires all Hughesnet customers to complete the form below prior to their installation date. On submitting this form, you are agreeing to allow EZTV to agree to the HughesNet ™ Subscriber Agreement on your behalf.

This agreement does not change or supercede any agreement between you, the customer, and HughesNet. By completing and submitting the form below you are placing an Installation Order for a residential or SOHO HughesNet Satellite Internet System. You have read the HughesNet Subscriber Agreement, and agree to all the terms and conditions contained therein.
Limited Installation
EZTV will provide an installation limited to the following:
- Up to 100 feet of dual coax cable installed between dish and inside satellite modem.
- Installation of dish on an exterior wall no more than 8 feet above finish grade. Roof installations and ground pole mounts subject to additional charges.
- Routing of cable on exterior of home following our Worker Safety and Cable Routing Policy.
- Crawl space and Attic work not included. and subject to additional charges.
- Installation of all HughesNet equipment and connection to computer, co-located with satellite modem.
- Verification that computer can browse the Internet. Instructions on setting up HughesNet Customer Account.
- Instructions on setting up email. EZTV is not responsible for creating email accounts.
- EZTV is not responsible for installation of, or configuration of routers, hubs, or switches. Any labor or materials required to install and configure these devices is subject to additional charges.
